Job summary

High Point & Affiliated Organizations is recruiting for a Recovery Support Navigator at our Community Based Recovery Services (CBRS) in Plymouth, MA. The role focuses on client-centered case management, transportation coordination, and connections to medical and counseling services within a recovery-oriented framework.

You will work with diverse clients and families, maintain strict confidentiality, complete documentation, and participate in mandatory trainings.

Qualifications

• Bachelor’s degree or lived experience of substance use and recovery.
• Valid driver’s license and access to a car.
• Preference for 1+ year related experience.

Responsibilities

• Ensure compliance with HIPAA and 42 CFR Part 2.
• Provide client-centered case management and CSP services.
• Coordinate with outpatient units and CSP sign-ups.
• Maintain safety and supervise client/staff in the program.
• Document needs assessments, service plans, and encounter notes.
• Maintain productivity and billable hours.
• Collaborate with outside providers as indicated.
• Respond to crises and provide 24-hour on-call coverage.
