Job summary

High Point & Affiliated Organizations is seeking a Family Support Navigator for the Youth Point program in Brockton, MA. The role will support open access screening for mental health and substance use services, assist families with intake, and coordinate care transitions within the CBHC framework.

The ideal candidate has caregiver experience, a psychology-related degree (preferred) or equivalent, and strong interpersonal skills. Bi-lingual applicants are encouraged.

Qualifications

Family Support Navigator

Program/Location: Youth Point / Brockton
Education/Licensure: High School Diploma/GED; Associate's degree in human services preferred
Pay Rate: $22-$27/hour
Status: Full Time
Shifts: Flexible Schedule, Mostly Monday-Friday; occasional weekend hours required

Family Support Navigator Qualifications
  • Preference of someone withlivedexperienceas a caregiver and 1 yearexperiencewith navigating the system
  • Bachelor’s Degree level degree in Psychology or a related human services field (preferred), or equivalent experience
  • 2 years of experience in behavioral health and/or inpatient psychiatric setting
  • Bi-lingual/Multi-lingual applicants are strongly encouraged to apply

  • Maintain mental alertness and attention to high detailMaintain a neat and professional appearance and provide excellent customer service as needed
  • Follow established security protocols while using initiative and good judgement without bias
Family Support Navigator Duties & Responsibilities
  • Help to support an open access model for screening and assessments for mental health, substance use, and co-occurring treatment services within the CBHC service model
  • Provide initial screenings and assist patients and families with accessing CBHC services
  • Assist patient with intake paperwork and ensure proper release and consent forms are complete
  • Participates in patient outreach and engagement to support access to treatment
  • Coordinates patient transportation services, as needed
  • Provide care coordination and discharge planning activities to support transitions of care
  • Maintain and document collateral contacts with other providers as needed, including, but not limited to primary care physicians, probation, parole, DCF, family members, natural supports, etc.
  • Coordinate with local Emergency Services Provider for concerns related to imminent psychiatric risk and hospitalization, referral support for CBHC core services, and follow-up
  • Engage in community outreach activities as needed
  • Must have experience as a caregiver or parent of a youth with social-emotional needs.
  • Educates caregivers on how to access community resources and navigate child-serving systems.
  • Works directly with caregivers to provide support, education and guidance.
  • Perform other duties as assigned; including providing security support for the program to monitor patient and staff safety as part of their duties
Family Support Navigator Requirements
  • Must present as courteous and professional at all times
  • Good case management skills
  • Experience working in the mental health/substance use field
  • Demonstrated flexibility and ability to perform multiple tasks
  • Must have excellent communication skills, including the ability to organize ideas in logical and clear fashion
  • Must have the ability to work with others in cooperative and collaborative manner
  • TB screening
  • Certification in the HPTC approved de-escalation program
  • Strong and effective verbal, written and organizational skills
  • Knowledge of basic computer skills and software programs including the ability to learn, use and train HPTC`s electronic medical records
  • Ability to provide leadership and program development within an outpatient site
  • Knowledge of major clinical therapeutic models and behavioral health best practice
  • Strong organizational, verbal and written communication skills
