Job summary

Jericho Project is seeking a Care Manager for the Family Supportive Scatter-site Apartment program in the Bronx. You will manage a caseload of about 15 formerly homeless families, with frequent tenant contact and regular meetings to support housing stability.

The role emphasizes trauma‑informed, culturally responsive engagement, collaboration with providers, and adherence to funder requirements. Field work can be up to 85% of time, with supervision and training as part of ongoing professional

Qualifications

• Bachelor’s degree in psychology, child development or social work.
• At least two years of experience with persons with severe mental illness.
• Two years of experience working with children with development needs.
• Familiarity with Harm Reduction, Housing First, and Motivational Interviewing.
• Bilingual Spanish/English preferred.

Responsibilities

• Provide on-site and field case management, including mental health assessments and service planning.
• Frequent contact with families with weekly/monthly meetings and aftercare contacts.
• Develop and update Individual Service Plans and document progress.
• Coordinate with community providers and housing staff; serve as liaison.
• Lead life-skills groups and support services; assist with referrals.
• Ensure compliance with organizational and funder regulations and complete documentation as required.
• Field work up to 85% of time.
