Job summary

Comunilife Inc. is seeking experienced Case Managers for the HRA Scatter Program in the Bronx. You will manage a caseload of about 22 clients, complete assessments, and develop service plans with home visits and fieldwork required.

Qualifications

• Bachelor’s or Associate degree with relevant case management experience.
• Knowledge of HRA Congregate Housing Program guidelines.
• Direct service experience with clients who are homeless, HIV/AIDS, or with substance use.
• Strong written and oral communication; proficient with Google Workspace and MS Office.

Responsibilities

• Complete comprehensive assessments and service plans.
• Initiate and monitor referrals and services for clients.
• Provide psychoeducation on topics such as substance abuse and entitlements.
• Lead individual and group workshops.
• Collaborate with interdisciplinary teams and document services.
