Job summary

St. Joseph Center in Los Angeles is seeking a Scattered Site Case Manager to provide high‑quality case management for individuals experiencing homelessness as they pursue permanent housing.

You will work with housing program staff to assess eligibility for subsidized housing, conduct regular home visits, coordinate services, and support clients with budgeting, transportation, and community referrals while maintaining HMIS/CHAMP notes.

Qualifications

• Bachelor’s degree in social work, human services, or a related field; combination of education and experience may be substituted.
• One to two years of experience serving highly vulnerable populations.

Responsibilities

• Provide high quality case management services to clients navigating from homelessness to tenancy within the community.
• Evaluate housing eligibility with housing program staff and assist with subsidized housing applications.
• Maintain regular face‑to‑face case management sessions and caseload management across scattered sites.
• Prepare housing voucher documentation and annual recertification with housing authorities.
• Conduct on‑site hours at assigned properties and initial/ongoing home visits.
• Coordinate referrals to health, mental health, education, and vocational services; advocate with other orgs as needed.
• Develop individualized service plans addressing housing, financial stability, education, mental health and related risk factors.
