Job summary

Jericho Project is seeking a Care Manager for the Family Supportive Scatter-site Apartment program in the Bronx. You will carry a caseload of about 15 formerly homeless families, performing on-site and field-based care management with regular contact, including meetings and phone check-ins.

Responsibilities include developing service plans, coordinating with mental health and housing resources, documenting progress, and ensuring compliance with funding requirements.

Qualifications

• Bachelor’s degree in psychology, child development, social work or related field.
• Experience working with persons with mental illness preferred.
• Experience with homeless population a plus.
• Strong written and verbal communication skills required.

Responsibilities

• Provide direct care management services on-site and in the field with a focus on housing stability.
• Maintain accurate and up-to-date participant files and documentation.
• Use evidence-based models (Motivational Interviewing, Critical Time Intervention, Harm Reduction).
• Coordinate with clinical teams and community providers to develop service plans.
• Engage families with regular meetings, aftercare, and referrals as needed.
