Job summary

Health Connect America is seeking a Wraparound Facilitator to support children, youth, and families with individualized care plans and resources to promote long-term stability. You will collaborate with families, schools, and community partners to coordinate services and ensure effective wraparound processes.

The role emphasizes caregiver involvement, team leadership, and advocacy, with travel requirements to cover local and statewide areas.

Qualifications

• High school diploma or GED required; Bachelor's degree in social or behavioral sciences preferred.
• Must self-identify as the primary caregiver of a child/youth with mental, emotional, behavioral, co-occurring, and/or autism spectrum disorder who has navigated child-serving systems to access treatment and resources.
• Experience working with children/youth and families with significant behavioral health needs.

Responsibilities

• Coordinate and lead a collaborative wraparound team to help youth and families achieve goals.
• Conduct outreach and intake with newly referred families and complete the intake process.
• Engage the family and assemble the child and family team, including caregivers and service providers.
• Schedule and facilitate child and family team meetings at least every 30 days and document plans.
• Ensure MI-informed wraparound processes are conducted with high quality and fidelity.
