Health Connect America is seeking a Wraparound Facilitator to support children, youth, and families by developing individualized plans that leverage strengths and connect them to resources for long-term stability. You will collaborate with a broad network of providers, schools, and community partners to implement care plans with fidelity and advocate for essential services.

This role emphasizes family-centered care, effective coordination, and hands-on engagement in local communities.

Qualifications

• High school diploma or GED; Bachelor's degree preferred.
• Must self-identify as the primary or designated caregiver of a child/youth with a mental, emotional, behavioral, co-occurring, and/or autism spectrum disorder.
• Experience working with children/youth and families with significant behavioral health needs.
• Complete training recognized by the Tennessee Department of Mental Health and Substance Abuse Services within 6 months; CFSS certification within 1 year.

Responsabilités

• Communicate with clients, colleagues, and others to answer questions, disseminate or explain information, and address complaints/concerns.
• Conduct outreach and engagement with newly referred families, including initial phone contact and intake completion.
• Engage the family and assemble the child and family team with youth, caregivers, service providers, and supports.
• Coordinate and facilitate child and family team meetings at least every 30 days; document plan of care and revisions.
• Ensure MI-informed wraparound process is conducted with high quality and fidelity.
• Draw on caregiver experience to provide caregiver-to-caregiver support.
• Identify and engage local community resources with regional Community Engagement Specialists.
• Provide SOC 101 trainings to caregivers, new staff, and partners.
• Travel approximately 75% of the time, mostly local and statewide.

Formation

High school diploma or GED

Bachelor's degree in social or behavioral sciences preferred
