Job summary

Health Connect America, Inc. seeks a dedicated caregiver to support families navigating behavioral health services. You will engage with families, coordinate teams, and ensure care plans are comprehensive and actionable.

Qualifications

• High school diploma or GED equivalent required. Bachelor’s degree in social or behavioral sciences preferred.
• Must self-identify as primary or designated caregiver of a child/youth with mental, emotional, behavioral disorders who navigated services.
• Experience with children/youth and families with behavioral health needs.
• Complete TDMHSAS training within 6 months; CFSS certification within 1 year of employment.

Responsibilities

• Communicate with clients, colleagues, and others to answer questions and address complaints.
• Conduct outreach with newly referred families and complete intake processes.
• Assemble the child and family team including youth, caregivers, and supports.
• Coordinate and facilitate family team meetings at least every 30 days and document plans.
