Job summary

Health Connect America in Tennessee is seeking a Wraparound Facilitator to lead and coordinate family-centered teams for children and families, building strength-based plans and connecting resources to promote stability and success.

You will conduct intake, engage families, schedule monthly team meetings, and travel about 75% locally and statewide, applying MI-informed wraparound processes with strong organizational and communication skills.

Qualifications

• High school diploma or GED required; bachelor's degree preferred.
• Must identify as the primary or designated caregiver of a child or youth with special needs.
• Experience with children/youth and families with behavioral health needs.
• Complete resiliency-based caregiver training within 6 months of employment.
• CFSS certification within 1 year of employment.

Responsibilities

• Coordinate and lead the child and family team meetings at least monthly.
• Engage families, complete intake processes, and build individualized plans.
• Provide caregiver-to-caregiver support and connect to community resources.
• Travel approximately 75% of the time locally and statewide.
