River Oak Center for Children is seeking a dedicated Wraparound (FSPW) professional in Elk Grove to provide intensive, family-centered mental health services to children and youth aged 0–21 in homes, schools, clinics, and community settings.

You will collaborate with treatment teams, lead CFT meetings, coordinate care plans, and ensure HIPAA-compliant documentation while supporting families to achieve goals and navigate community resources.

Qualifications

• Masters or higher in social work, psychology, counseling, or closely related field.
• 2 years full-time mental health or social services experience in relevant settings.
• Bachelor’s degree with 4 years of experience in mental health or social services.
• Associate degree with 6 years of experience in mental health or social services.

Responsabilités

• Coordinate individualized care with youth, families, treatment teams and community partners.
• Develop, review, and coordinate care plans and client goals.
• Lead Child and Family Team (CFT) meetings and coordinate services.
• Conduct home, school, office, and community visits for assessment and support.
• Provide individual and family support to build strengths and achieve goals.
• Provide crisis support and participate in on-call duties as required.
