Job summary

Health Connect America, INC seeks a Family Centered Treatment Specialist to deliver evidence-based FCT services to children, adolescents, and families in home, clinic, and community settings. You will apply the FCT model with fidelity, develop Service Plans with the Treatment Team, and support child and family goals.

Qualifications include a Bachelor’s degree in human services or related field, two years’ experience with youth or families, and FCT Certification preferred.

• Bachelor’s degree in human services or related field required.
• Two years’ experience working with youth or families required.
• FCT Certification preferred and must be completed within one year of employment where applicable.

Responsibilities

• Provide Family Centered Treatment (FCT) services to children, adolescents and families.
• Deliver 24/7 on-call coverage to a caseload of families and children.
• Develop and coordinate Service Plans with the Treatment Team.
• Conduct interventions with families aligned with the FCT model.
• Coordinate discharge planning and link to community resources.
• Monitor and document progress; participate in QA/QI activities.
