Job summary

Health Connect America seeks a Family Centered Treatment Specialist to deliver evidence-based services in homes and communities. You will guide children, adolescents, and their families through the structured FCT model and work with a multi-disciplinary Treatment Team to ensure holistic care.

Successful candidates typically hold a Bachelor's in human services, two years of relevant experience, and a preference for FCT certification.

Qualifications

• Bachelor's degree in human services or related field.
• Two years’ experience with youth or families.
• Current Family Centered Treatment Certification is preferred.

Responsibilities

• Deliver FCT services at home and in community settings.
• Guide families through four phases of the FCT model.
• Coordinate with Treatment Team across medical, behavioral health and schools.
• Develop and refine individualized Service Plans with families.
• Document care in the Electronic Health Record and support QA activities.
