Job summary

Health Connect America, Inc. is seeking a Family Centered Treatment Specialist to deliver evidence-informed care to children, adolescents, and families in home, community, and office settings.

The role emphasizes fidelity to the FCT model, development of individualized Service Plans, and collaboration with Treatment Team members to ensure coordinated services. Qualifications

• Bachelor’s degree in human services or related field required
• Master’s degree in human services or related field preferred

Responsibilities

• Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements.
• Deliver FCT services to children, adolescents and their families, primarily in their living environments, with a family focus.
• Provide 24/7 on-call coverage to a caseload of families and children.
• Develop and coordinate discharge plans based on treatment needs and aftercare plans.
• Coordinate and oversee initial and ongoing assessment activities and Service Plan implementation.

Education

Bachelor's degree in human services or related field

Master's degree or higher in human services (preferred)
