Job summary

Health Connect America is seeking a Family Centered Treatment professional to deliver FCT services to children, adolescents, and their families in home and community settings. You will work with the treatment team to develop and maintain Service Plans, provide psychoeducation, and ensure timely documentation in the Electronic Health Record.

The role requires a Bachelor’s degree in a human services field, 2 years’ relevant experience, and FCT certification within one year.

Qualifications

• Current Family Centered Treatment Certification is preferred in all areas.
• Virginia QMHP or eligibility to be QMHP with Virginia Board of Counseling.
• Bachelor’s degree in human services or related field required.
• Two years’ experience working with youth or families.
• FCT Certification must be completed within one year of employment.

Responsibilities

• Provide Family Centered Treatment (FCT) Services per service definition and model fidelity requirements.
• Deliver FCT services to children and families in home and community settings with a family focus.
• Provide 24/7 on-call coverage to a caseload of families and children.
• Develop and coordinate Service Plans with the treatment team and document progress in the Electronic Health Record.
• Coordinate discharge planning and linkage to community resources.
