Job summary

As part of an interdisciplinary team, you will help families develop plans of care, connect with providers and resources, and document work in the electronic health record while meeting program requirements.

Qualifications

• Bachelor’s level degree in a behavioral health field preferred.
• Or 5 years experience in a related role.

Responsibilities

• Establish rapport with families, schools and providers.
• Engage, educate, and empower families to foster success.
• Collaborate with interdisciplinary team for client care.
• Maintain active caseload and document work in the EHR.
