• Collaborate with an interdisciplinary team to plan care.
• Document work in the electronic health record.
• Maintain an active caseload to meet annual program needs.
• Meet billable expectations as directed by the supervisor.
• Conduct home visits and connect families with resources.
• Ensure documentation is provided in the family’s preferred language (Spanish).
