Terros Health is seeking a Families FIRST - Care Coordinator at the Stapley Rd Health Center in Mesa, AZ. This full-time role supports families involved with the Department of Child Safety, coordinating services to overcome barriers to reunification while working with DCS and clinical teams.

The position emphasizes professional, friendly, self-starting, organized and compassionate behavior, with monthly service plan meetings and ongoing documentation.

Qualifications

• Must possess strong communication and interpersonal skills to work with families and partners.
• Ability to organize and coordinate multiple services and case plans.
• Demonstrated empathy and commitment to client-centered care.

Responsibilities

• Coordinate and finalize the Service Plan with the Service Team.
• Schedule monthly Service Plan reviews and report progress to DCS.
• Provide resources and support to families to achieve sobriety and stability at home.
• Maintain accurate progress notes and required documentation.
