Job summary

Terros Health is seeking a Families FIRST - Care Coordinator at the McDowell Rd Health Center in Phoenix, AZ. This full-time role supports families involved with the Department of Child Safety by coordinating services, records, and service plans with clinical teams and partners.

The position requires professionalism, compassion, organization, and the ability to work with DCS, Recovery Coaches, and community resources. Regular Monday–Friday hours and comprehensive benefits are provided.

Qualifications

• Professional, friendly, self-starter, organized and compassionate.
• Ability to drive to multiple locations across assigned territory.
• Experience coordinating with DCS, clinical teams and community partners.

Responsibilities

• Coordinate and support families to achieve case plan goals within set timelines.
• Schedule monthly Service Plan reviews with the Service Team and update plans quarterly.
• Engage patients and families, provide resources for housing, utilities, clothing, food, etc.
• Document progress and ensure records meet professional and state requirements.
• Collaborate with DCS, recovery coaches and other agencies to streamline services.
