Job summary

Terros Health in Mesa, AZ is hiring a Families FIRST Care Coordinator to support families navigating substance use barriers and reunification goals under DCS. You will work with the clinical team and community partners to coordinate services, schedule monthly goal reviews, and empower families toward stability.

The role requires an associate degree (or bachelor’s) in a human service field, strong communication skills, and proficiency with MS Office.

Qualifications

• Associates degree in human service or behavioral health; or a bachelor’s degree.
• At least three years of related experience in substance abuse, child welfare, or behavioral health.
• Proficiency with MS Office; NextGen a plus.

Responsibilities

• Coordinate and support DCS case plans with the clinical team.
• Provide monthly meetings to review goals and the Service Plan.
• Assist families with resources for housing, utilities, clothing, and more.
• Drive to various locations to visit patients within the assigned territory.
• Document progress notes and complete required records per guidelines.
• Attend team meetings (CFT, ART, TDM, SENSE, DCS/JOBS) as needed.
