Job summary

Apalachee Center, Inc. is seeking a FACT Therapist to join a dynamic multidisciplinary team in Tallahassee, FL.

The role focuses on delivering intensive community-based mental health treatment, rehabilitation, and self-management counseling for adults with severe mental illness. The ideal candidate will conduct comprehensive assessments, develop individualized service plans, and provide evidence-based therapies across office and community settings.

Qualifications

• Masters degree in counseling, social work, psychology, nursing, rehabilitation, special education, health education, or a related human services field.
• Two years of professional experience in providing services to persons with psychiatric and behavioral illness preferred.
• Professional experience working with persons with severe mental illness include Substance Abuse/Dependence disorders and Personality disorders preferred.
• Assertive Community Treatment (ACT) experience working in community setting consistent with the PACT model of service delivery preferred.

Responsibilities

• Conducts comprehensive mental health assessments and ongoing assessments of persons served mental illness, symptoms, status, and response to treatment.
• Schedules Individualized Treatment Team (ITT) meetings a month in advance of service plan expiring and reviews client progress with the treatment plan.
• Provide individual, group, and family therapy, utilizing evidence-based modalities and skills coaching to support recovery goals.
• Develop and update Client Service Plans with measurable goals and time frames.
• Documents progress notes in Avatar and ensures timely submission within 48 hours.
