Job summary

Apalachee Center, Inc. in North Florida seeks a compassionate FACT Substance Abuse Specialist to join our Florida Assertive Community Treatment team.

You will provide assessments, counseling, and recovery support to adults with co-occurring disorders, and coordinate care with a multidisciplinary team. Responsibilities include crisis intervention, treatment planning, connecting clients with resources, documenting care, and promoting stability and community integration.

Qualifications

• Assess mental health and substance use disorders in adults.
• Provide individual, group, and family counseling and support.
• Develop and monitor individualized treatment plans for recovery and stability.
• Coordinate with multidisciplinary team and community resources.

Responsibilities

• Conduct comprehensive mental health and substance use assessments.
• Provide individual, group, and family counseling and recovery support.
• Develop individualized treatment plans focused on recovery, relapse prevention, and long-term stability.
• Assist individuals with co-occurring mental health and substance use disorders.
• Provide crisis intervention and participate in scheduled after-hours on-call coverage.
• Connect clients with community resources, treatment programs, housing assistance, and other support services.
• Teach essential life skills and coping strategies.
• Collaborate with a multidisciplinary team to coordinate care and education.
