Job summary

Apalachee Center, Inc. is seeking a FACT Therapist to provide intensive community-based mental health treatment for adults with serious mental illness.

You will join a multidisciplinary team delivering evidence-based therapies, recovery-focused care, and CSP development to promote client autonomy and community inclusion. The role emphasizes assessments, treatment planning, cross-training, and collaboration with families, with responsibilities across office and field settings in the Tallahassee

Qualifications

• Master's degree from an accredited university or college in counseling, social work, psychology, nursing, rehabilitation, special education, health education, or related human services field.

Responsibilities

• Conduct comprehensive mental health assessments and monitor client status.
• Coordinate ITT meetings and review progress toward treatment goals.
• Provide individual, group, and family therapy using evidence-based approaches.
• Develop and update individualized Client Service Plans (CSP) with measurable goals.
• Document progress and ensure timely updates in patient records (Avatar).
• Support clients in developing skills for daily living and community integration.

Education
