Job summary

Atrius Health, part of the Optum family, seeks an experienced Nurse Practitioner or Physician Assistant to join its primary care team in Braintree, MA. Qualifications

• Minimum of three years independently managing a full primary care patient panel.
• Experience includes preventive care, longitudinal care and chronic disease management.
• Experience caring for adult patients ages 65 and older.
• Ambulatory primary care setting experience; not limited to episodic or specialty-only care.
• Experience inbox management, test results follow-up, patient messaging and referral coordination.

Responsibilities

• Independently manage a panel of primary care patients.
• Provide comprehensive preventive, acute, and chronic disease care.
• Collaborate with RNs, pharmacists, behavioral health, specialists, and population health teams.
• Focus on continuity of care, care coordination, and patient outcomes.
