Qualifications

Responsibilities

• Identify client support needs, plan services, and authorize payments.
• Schedule home visits and conduct assessments in clients' residences.
• Facilitate person-centered goal planning with clients and families.
• Complete and update client assessments and service plans within timelines.
• Identify crises and refer to appropriate authorities and services.
• Evaluate eligibility for enrolled programs and monitor progress.
• Maintain thorough case files and documentation.
• Collaborate with state services and community partners.
• Represent DDA in community meetings and professional settings.
