Qualifications

• Bachelor's degree in social sciences or allied field with two years of social services experience in developmental disabilities.
• Six years of experience in providing social services to people with developmental disabilities.
• 12 months as a Case Resource Manager Trainee.
• Graduate training can substitute, year for year, for one year of experience.

Responsibilities

• Identify client support needs, plan services, and authorize payments for appropriate services.
• Schedule and conduct home visits with clients and families.
• Lead person-centered goal planning and update assessments/service plans.
• Complete and revise client assessments within timelines for caseloads.
• Identify crisis situations and refer as needed to authorities or community resources.
• Determine and evaluate client eligibility for requested programs.
• Review, monitor, and document client plans and progress.
• Obtain translation/interpretation services when needed.
• Collaborate with community partners and state agencies; represent DDA professionally.
