Qualifications

• Requires prior professional case management experience; not entry-level.
• Experience providing services to adults with developmental disabilities.
• Ability to conduct person-centered service planning and home visits.
• Experience coordinating with state agencies and community partners.

Responsibilities

• Identify client support needs and plan/authorize payment for services.
• Schedule and conduct home visits in clients' homes.
• Facilitate person-centered goal planning with clients and families.
• Update assessments and service plans within timelines for caseload.
• Identify clients in crisis and refer to authorities or community resources.
• Determine and evaluate eligibility for programs.
• Review and monitor client plans, reports, and progress.
• Obtain translation/interpretation services when needed.
• Maintain complete case files and provider payment records.
• Collaborate with community partners and state services.
• Represent DDA in meetings and community events.
