Job summary

State of Washington’s Developmental Disabilities Administration seeks a skilled Resource Case Manager in Everett to provide specialized case management and resource support for people with developmental disabilities and their families.

You will conduct client assessments, facilitate goal-planning, coordinate with state agencies, and ensure services reflect person-centered practices. Travel within Washington State may be required; the role supports a hybrid work model.

Qualifications

• Bachelor's degree or equivalent social services experience.
• Minimum 2 years experience in social services for disabilities or related field.
• Graduate training or substitutions may apply.

Responsibilities

• Identify support needs of clients, plan, and authorize payment for services.
• Schedule and conduct face-to-face home visits with clients and families.
• Facilitate person-centered goal-planning discussions and update assessments/service plans.
• Complete and revise client assessments and service plans within timelines.
• Identify clients in crisis and refer to appropriate authorities or resources.
• Determine and evaluate eligibility for programs.
• Review and monitor client plans and progress.
• Obtain translation services when needed.
