Heartland Hospice is seeking an experienced Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. You will assess patients, coordinate with the IDG, and ensure continuity of care during nights, weekends, and on-call periods.

The role requires an active RN license, a nursing degree, and a strong background in hospice or related settings. You will lead field staff, manage documentation, and support regulatory compliance.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.

Responsabilités

• Provide direct in-person hospice care during after-hours shifts, including assessments, admissions, and visits.
• Assess physical, emotional, psychosocial, and spiritual needs of patients and families.
• Coordinate care with the Interdisciplinary Group (IDG) and adjust plans of care as needed.
• Educate patients and families on disease progression, symptom management, and hospice philosophy.
• Maintain accurate clinical documentation and ensure compliance with regulations.
• Lead and supervise field staff and participate in scheduling and workflow processes.
