Job summary

NewYork-Presbyterian Hospital in New York, NY is seeking a Patient Navigator for the Ambulatory Care Network to work in the Evening shift. You will guide patients through the continuum of care, educate them on navigating the healthcare system, and ensure a comprehensive discharge plan after ED visits.

You will connect patients with primary or specialty services, leverage community resources, and support improved health outcomes. Bilingual English/Spanish and strong communication are preferred.

Qualifications

• High School Diploma or GED.
• Bachelor's degree from an accredited college or university.
• Bilingual in English/Spanish preferred.
• Experience as a patient navigator in a hospital setting preferred.
• Proficient in Epic and Microsoft Office.

Responsibilities

• Coordinate the full breadth of the patient experience from ED to discharge.
• Educate patients on navigating the healthcare system and discharge planning.
• Provide non-clinical support by directing patients to primary or specialty care.
• Tap into internal and community resources to ensure optimal care and outcomes.
