NewYork-Presbyterian Hospital seeks a Patient Navigator for the Ambulatory Care Network (Emergency) evenings. You will guide patients through the care continuum, educate them on navigating the system, and support comprehensive discharge planning to ensure timely follow-up with primary or specialty care.

Responsibilities include directing patients to internal or community resources, coordinating care across departments, and maintaining clear communication to improve outcomes.

Competenze

• Bachelor's degree from an accredited college or university.
• Bilingual in English/Spanish is preferred.
• Proficient in Microsoft Office and other standard office tools.
• Experience as a patient navigator or similar role is preferred.

Mansioni

• Act as a patient ally to navigate the continuum of care and educate patients on navigating the healthcare system.
• Assist with discharge planning and connect patients to appropriate primary or specialty care.
• Coordinate with internal and community resources to ensure appropriate care and outcomes.
