NewYork-Presbyterian Hospital in New York, NY is hiring a Patient Navigator for the Ambulatory Care Network: Emergency, Evenings. You will guide patients through the continuum of care, educate them on navigating the healthcare system, and develop comprehensive discharge plans after ED visits.

Responsibilities

• Coordinate the full breadth of the patient experience by ensuring the patient is educated on navigating the healthcare system and has a comprehensive discharge plan after leaving the ED.
• Provide non-clinical support by directing patients to primary or specialty doctors or needed services.
• Connect patients with internal and community resources to ensure the best clinical care and outcomes.

Patient Navigator - Ambulatory Care Network: Emergency - Evenings

Serve as a patient ally throughout the continuum of care. Coordinate the full breadth of the patient experience by ensuring that the patient is well educated and knowledgeable on navigating the healthcare system and has a comprehensive discharge plan once leaving the ED. Then, go a step beyond by providing non-clinical support to our patients by directing them to a primary, specialty doctor or the service(s) that they need. Tap into internal and/or community resources, like neighborhood clinics, to ensure that patients get the best clinical care and outcomes.

The schedule for this position is Monday – Friday - 3:00 PM - 11:00 PM.

Preferred Criteria

• Experience working as a patient navigator, preferably in a hospital setting
• Bachelor's Degree from an accredited college or university
• Bilingual in English/Spanish
• Proficient in Epic

Required Criteria

• High School Diploma or GED
• Proficient in Microsoft Office
• Ability to manage high stress, chronically ill population with complicated psycho – social issues
• Diverse and culturally sensitive customer service skills
• Excellent oral and written communications skills
