Rappahannock Area Community Services Board is seeking an empathetic direct support professional to provide care and supervision for adults with developmental disabilities in their home and community settings. You will assist with personal care, household tasks, and social activities following a plan of care.

The role requires evening, overnight, holiday, and weekend shifts on a rotating schedule, documentation of progress, communication with the team, and collaboration with families and service

Qualifikationen

• High School Diploma or GED is required.
• Some experience with developmental disabilities preferred.
• Pre-employment drug screening and background check required.

Aufgaben

• Provide support and supervision to adults with developmental disabilities.
• Assist with personal care, household tasks, and meals.
• Administer and document medications.
• Monitor health and safety needs, and report changes.
• Coordinate transportation and accompany to appointments.
• Document daily progress notes and communicate with the team.
• Liaise with families and other service providers.
• Follow each resident's plan of care and participate in activities.
