Job summary

Rappahannock Area Community Services Board (RACSB) provides support and supervision to adults with developmental disabilities in their home and in community activities, following an individualized plan of care. The role includes documenting progress and communicating health and safety needs to the team.

Shifts include evenings, overnights, holidays, and weekends on a rotating basis, with responsibilities for medication administration and liaison with families as needed.

Qualifications

• Some experience working with adults with developmental disabilities and/or mental illness preferred.
• Knowledge of person-centered practices and techniques.
• Knowledge of common housekeeping practices (cleaning, cooking, laundry).
• Ability to communicate effectively, orally and in writing.

Responsibilities

• Supports residents with basic needs including self-care, bathing, and toileting; household chores and meal planning/preparation, modeling appropriate social behavior; communication; basic financial management, and integrating with others outside of the home.
• Supports residents with and in social and leisure activities inside and outside the home.
• Assists residents with physician appointments.
• Assists with transporting residents.
• Administers and documents the administration of resident medications.
• Provides awake overnight supervision to residents, responding to resident needs as necessary during shift.
• Greets visitors and supervises visitation in the home.
• Follows each resident's plan of care in collaboration with resident, his/her family, and other service providers, as appropriate.
• Documents daily progress notes.
