Job summary

Luminos Hospice is seeking a skilled on-call RN for weeknights to deliver quality nursing care in patients' homes. You will coordinate with the interdisciplinary team, families, and referring agencies to ensure seamless care and timely documentation.

The role requires current RN licensure, driver’s license, and CPR certification, with hospice experience preferred. Join a compassionate team dedicated to exemplary end-of-life care.

Responsibilities

• Provide nursing care to patients in home or residence during weeknight on-call shifts.
• Coordinate care with Interdisciplinary team, patients, families, and referring agency.
• Administer nursing care as per physician orders and clinical protocols.
• Assume responsibility for coordination of care and timely documentation.
