Job summary

Luminary Hospice is seeking an after-hours registered nurse to support patients in their homes during evening and overnight shifts. The role includes a rotating schedule: one week on, one full week off, with weekend coverage Friday 5:00 PM to Monday 8:30 AM.

The RN will assess, plan, and implement care, administer medications, educate families, and coordinate with the interdisciplinary team after hours. Requires active RN license and CPR.

Qualifications

• Bachelor's or Associate's degree in nursing or related fields.
• Active nursing license in good standing.
• Active CPR certification.
• 2–5 years of experience in healthcare management, hospice, or palliative care, preferred.
• 1 year Medicare Hospice Certification and CHAP guidelines experience, preferred.
• Knowledge of hospice care and interdisciplinary teamwork.
• Proficiency in MS Office and software systems.

Responsibilities

• Complete initial and comprehensive patient and family assessments.
• Provide nursing care using the nursing process and plan of care.
• Evaluate patient status and update care plans as needed.
• Develop goals based on nursing diagnosis with patient/family involvement.
• Administer medications and treatments per physician plan.
• Assist with activities of daily living and support self-care.
• Coordinate after-hours patient care with case managers.
