Job summary

Luminary Hospice in Columbus, OH is seeking an experienced on-call nurse to support patients after hours within home and community settings. This role requires applying nursing processes, coordinating with the hospice interdisciplinary team, and ensuring safe, compassionate end-of-life care.

You will complete assessments, develop and revise care plans, administer medications, educate families, and document all care activities while working closely with Case Managers and physicians to meet

Qualifications

• Bachelor's or Associate's degree in nursing or related field.
• Active nursing license in Ohio in good standing.
• CPR certification required.
• 2–5 years in healthcare/hospice or palliative care preferred.
• Experience with Medicare Hospice Certification and CHAP preferred.
• Strong organizational and communication skills.

Responsibilities

• Completes initial and comprehensive assessments of patient and family.
• Provides professional nursing care using the nursing process.
• Assesses status and initiates plan of care, revising as needed after hours.
• Develops care plans with goals based on nursing diagnosis.
• Administers medications and treatments as prescribed.
• Assists with activities of daily living and comfort care.
• Coordinates patient care with Case Managers after hours.
• Documents clinical notes and pain/symptom changes.
• Communicates with physicians about patient needs.
• Educates patients and families on self-care and health counseling.
• Collaborates with hospice team to meet emotional needs.
• Participates in quality improvement activities.
• Performs additional duties as assigned.
