Illumination Health + Home is seeking a Medical Coordination Assistant to ensure seamless medical care for clients discharged from hospitals or contracted facilities. The role focuses on coordination of follow-up care and social support in Recuperative Care Centers.

Schedule is Sunday-Thursday, 3pm-11:30pm. Pay ranges from $20-24 per hour, with swing and graveyard differentials. Strong communication, HIPAA compliance, and bilingual abilities are valued assets.

Qualifications

• High School Diploma or equivalent is required.
• Basic computer skills including email and data summarization in spreadsheets.
• Bilingual in English and Spanish is a plus.

Responsabilités

• Perform initial intakes to ensure proper discharge from hospital and document findings.
• Assess medical and mental health status upon arrival and coordinate follow-ups with doctors.
• Conduct morning, afternoon, and evening checks for all assigned clients.
• Link clients to medical services (insurance, PCP, specialists, referrals).
• Coordinate home health visits and follow-up MD appointments.
• Promote health literacy with patient education on illnesses, medications, and prevention under supervision.
• Provide Exit Reports with current medications, future appointments, and insurance information.
• Arrange transportation for medical appointments as needed for client advocacy.
• Report emergencies to Supervisor/Manager and initiate appropriate medical response.
• Support client recuperation by providing needed items and maintaining facility cleanliness.
