Job summary

Illumination Health + Home is seeking a Medical Coordination Assistant to ensure smooth medical follow-up for formerly hospitalized clients in recuperative care centers. You will perform intakes, assess needs, arrange follow-ups, and coordinate transportation while educating clients on health and medications.

The role requires HIPAA-compliant care and strong teamwork. The position runs Sunday through Thursday, 11:00pm–7:30am, offering $20–$24/hour with swing and graveyard differentials.

Qualifications

• High School Diploma or equivalent.
• Basic computer skills including email and data summaries.

Responsibilities

• Perform initial intakes for proper discharge and care planning.
• Assess medical/mental health status and arrange follow-up with doctors.
• Coordinate home health visits and follow-up MD appointments.
• Educate clients on health, medications, and preventive care.
• Arrange transportation for medical appointments as needed.
• Provide exit reports with meds, appointments, and insurance details.
• Maintain confidentiality and HIPAA compliance.
