Job summary

Hospice of The Bluegrass, Inc. is seeking a Home Care CNA for PACE participants. You will provide direct personal care, assist with activities of daily living, light housework, meal prep, and ensure safety and comfort in the home setting.

Under RN supervision, you will identify changes in participant condition and report them to the Interdisciplinary Team, communicating clearly while working independently in the field to support independence and quality of life.

Qualifications

• High school diploma or GED required.
• Must be at least 18 years old.
• Current SRNA certification required.
• Completed approved CNA training program.
• Minimum 1 year of experience with frail/elderly or training upon hire.
• Food handling permit obtained upon hire.
• CPR certification kept current.
• Ability to work independently with minimal supervision.

Responsibilities

• Provide direct personal care and supportive services to participants in their homes.
• Deliver one-on-one care in a home setting, including ADLs, light housekeeping, and meal prep.
• Observe participant condition and report changes to RN for timely intervention.
• Maintain communication with Interdisciplinary Team (IDT) and coordinate care.
• Adapt to changing participant needs and schedules.
