Job summary

Bluegrass Care Navigators seeks a Home Care CNA to deliver one-on-one personal care in participants' homes, supporting independence and safety. You will assist with daily activities, light housekeeping, and meals, while monitoring participant condition and reporting changes to the RN and IDT.

Under RN supervision, you’ll work independently in the field, communicating clearly with the care team and families to ensure continuity of care and a high quality of life for seniors in our PACE program.

Qualifications

• High school diploma or GED required.
• Current SRNA certification in Kentucky required.
• Completed approved CNA training program.

Responsibilities

• Provide direct personal home care one-on-one with participants.
• Assist with activities of daily living, safety and quality of life.
• Light housekeeping, meal preparation, and observation of participant condition.
• Document changes and report to Interdisciplinary Team (IDT).
