Job summary

Bluegrass Care Navigators in Frankfort, KY, seeks a Clinical - BCN PACE Center Certified Nursing Assistant to provide direct personal care to participants at the PACE Center, under RN supervision. This role supports activities of daily living, promotes safety and independence, and helps with transportation as needed.

Strong communication and compassionate care are essential, with accurate documentation to meet regulatory requirements. Schedule: Monday–Friday, 8am–4:30pm.

Qualifications

• High school diploma or GED required.
• Current SRNA certification through Kentucky Board of Nursing required.
• Completion of an approved CNA training program required.
• Minimum of one year of experience working with frail and elderly population.
• Food handling permit required (to be obtained upon hire).
• Ability to work independently with minimal supervision and adapt to changing participant needs and schedules.
• Demonstrated ability to provide compassionate care and effectively communicate with adults and elderly populations.
• Ability to read, understand, and follow written and verbal instructions, policies, and safety procedures.
• Ability to complete accurate documentation and records.
• Ability to collaborate effectively with PACE staff and IDT members.

Responsibilities

• Provide direct personal care and supportive services to PACE Center participants.
• Assist with activities of daily living and promote safety and independence.
• Observe and report changes in participant conditions.
• Maintain clean and safe environment and ensure compliant documentation.
• May assist with participant transportation as needed.
