Job summary

Bluegrass Care Navigators seeks a Clinical - BCN PACE Center Certified Nursing Assistant in Lexington, KY to provide direct personal care to PACE Center participants under RN supervision. You will assist with activities of daily living, promote safety and independence, and document changes in participant conditions. Some transport assistance may be needed.

Qualifications

• High school diploma or GED required; must be 18+.
• SRNA certification through Kentucky Board of Nursing required.
• CNA training program completion required.
• Minimum of one year experience with frail and elderly population.
• Food handling permit required.
• Must be a licensed driver.

Responsibilities

• Provide direct personal care and supportive services to PACE Center participants under RN supervision.
• Assist with activities of daily living, promote safety and independence, and support engagement in center-based activities.
• Observe and report changes in participant conditions; maintain clean, safe environment; ensure accurate documentation per regulatory requirements.
• May assist with participant transportation as needed.
