Job summary

Bluegrass Care Navigators is seeking a PRN PACE Home Care CNA to support participants in their homes and at the PACE Center. You will assist with bathing, dressing, toileting, meal prep and feeding, monitor vital signs, and document intake and output.

The role requires compassionate, attentive care, adherence to safety protocols, and ability to collaborate with the Interdisciplinary Team, providing reports on changes to clinical staff.

Qualifications

• Must be 18+ with a High School Diploma or GED.
• State Registered Nurse Aide through the Kentucky Board of Nursing.
• Proof of completion of a recognized CNA training program.
• At least 1 year of experience with the frail/elderly or proper training on hire.
• Food handling permit to be completed upon hire (if applicable).
• Current CPR certification.
• Ability to read and interpret safety and operating instructions.
• Ability to write routine reports and correspondence.
• Ability to work with PACE staff and the Interdisciplinary Team.
• Ability to speak effectively before groups; follow written/oral instructions.
• Must have a valid driver’s license and insured vehicle.

Responsibilities

• Assist with bathing, dressing, toileting, and daily living activities.
• Prepare meals and assist with feeding; monitor intake/output.
• Monitor vital signs and report changes to clinical staff.
• Maintain a clean, safe home environment and perform routine housekeeping.
• Transport and assist with simple errands as needed.
• Communicate with participants and team members in a compassionate manner.
