Responsibilities

• Record vital signs and document data in electronic records.
• Support residents with activities of daily living per care plans.
• Monitor changes in patient conditions and report to licensed nurses.
• Assist with therapies and range of motion as directed by RNs/LPNs.
• Maintain infection control and clean, safe spaces in resident rooms and common areas.
• Assist with transfers, positioning, and transportation of residents.
