Job summary

Pine Rest Christian Mental Health Services in Grand Rapids, MI is seeking a Bachelor’s Level Clinical Coordinator to join our 151 Contact Center. You will assess patient needs, coordinate care, handle insurance information, and support discharge planning within a compassionate, faith-based environment.

The role requires a Bachelor's degree in Social Work, Psychology or related field and at least one year of related experience.

Qualifications

• Bachelor’s degree in Social Work, Psychology or related field required.
• A minimum of one year of related work experience preferred to direct workflow and to effectively prioritize workload.

Responsibilities

• Collaborate with community agencies on behalf of clients and/or the organization.
• Professionally and calmly handle crisis calls, responding to acuity and needs of internal and external customers.
• Coordinate workflow and participate effectively in a multi-disciplinary team to ensure excellent patient experience.
• Establish therapeutic relationships with assigned patients/clients including engaging, deescalating, screening, and managing crises.
• Liaise between community resources, payers, family, patient/client, and hospital- or clinic-based providers of care.
• Document and report all necessary information according to established policies and procedures when applicable.
