Job summary

Qualifications

• Bachelor’s degree in Social Work, Psychology or related field required.
• Minimum of one year of related work experience preferred, to direct workflow and prioritize workload.

Responsibilities

• Collaborate with community agencies to advocate for clients and the organization.
• Professionally handle crisis calls, assessing acuity and needs of internal and external customers.
• Coordinate workflow and participate in a multidisciplinary team to ensure excellent patient experience.
• Establish therapeutic relationships, engage and deescalate, screen and manage crises.
• Liaise between community resources, payers, family, patient and care providers.
• Document and report information according to policies and procedures.
