Job summary

Pine Rest Christian Mental Health Services in Grand Rapids, MI, seeks a Clinical Social Worker to join a dedicated team delivering individualized treatment plans. You will provide individual, group, and family therapy, coordinate with families, referrals, and insurers, and guide patients toward appropriate levels of care.

Requirements include a Master’s degree in social work (LLMSW or LMSW) and current Michigan licensure; one year of mental health experience is preferred.

Qualifications

• Master’s degree in social work (LLMSW or LMSW) required.
• Michigan licensure in social work required.
• One year mental health experience preferred.
• Experience with discharge planning and risk assessment advisable.

Responsibilities

• Assess and prioritize patient treatment needs including discharge planning.
• Implement plan of care and collaborate with the clinical team.
• Provide individual, group, and family therapy and psychosocial assessments.
• Serve as liaison between community resources, families, patients, and providers.
• Monitor safety and report potential risks to leadership.
• Uphold Pine Rest mission and promote diversity and professional excellence.
