Job summary

Pine Rest Christian Mental Health Services in Grand Rapids, MI seeks an Assessment & Referral Clinician to provide triage, assessment and referral services for patients in crisis. The role involves collaborating with community agencies and coordinating admissions and referrals to appropriate care levels.

The position requires MI licensure (LLP/LMSW/LLMSW), culturally inclusive care, and strong interpersonal skills to work with patients, families and a multidisciplinary team.

Qualifications

• License in MI as LLP, LMSW or LLMSW.
• Certification commiserate with treatment provided.
• Ability to provide culturally inclusive care.

Responsibilities

• Triage, assessment and referral for patients in crisis.
• Coordinate admissions/intakes with insurance for authorization.
• Refer patients to outpatient services as needed.
• Ensure documentation in medical records complies with protocols.
• Respond to range of client needs using differential diagnoses and treatment approaches.
