Qualifikationen

• Active, unrestricted Registered Nurse (RN) license in Connecticut.
• Minimum 1–2 years nursing experience in SNF/LTC or sub-acute rehab preferred.
• Experience handling admissions, discharge coordination or unit management is desirable.
• Hands-on experience with EMR systems (e.g., PointClickCare) for quick chart initiation.
• Strong organizational, prioritization, and communication skills to build trust with families and hospital staff.

Aufgaben

• Lead evening intake and onboarding for admissions (3:00 PM–11:00 PM).
• Perform initial nursing assessments, skin evaluations, and admission documentation.
• Reconcile discharge summaries, orders, and pharmacy profiles to ensure medication continuity.
• Collaborate with discharge planners, care teams, and families for room setups and equipment.
• Initiate preliminary care plans and communicate needs to on-duty supervisors.
• Orient new residents and families to facility policies and routines.
